Leeds GP Confederation holds Confed Connect sessions to engage with and inform general practice colleagues on practical and relevant topics. Our first Confed Connects session was hosted by Dr Chris Mills on 21st April and focused on frailty. If you missed it, you can access the slides and watch the replay here (please use Microsoft Edge). We are continuing this mini-series focused on frailty in our next two Confed Connects sessions, which have been designed in partnership with the CCG. Read on for more details.

Confed Connects – Lunch & Learn session on MDT working –  Thursday 26th May 2022, 1-2pm

This is the second in a Confed Connects series of events on Frailty aimed at supporting practices to implement the second year of the frailty local enhanced service (LES) and prepare for the Anticipatory Care direct enhanced service (DES). Dr Chris Mills will host this session on MDT working, which will be a conversation with Geriatrician,  Neighbourhood Team and Mental Health colleagues to agree an approach to ‘MDT working’ at neighbourhood level, that works for all across the city. At the heart of proactive care for people living with frailty is the well co-ordinated efforts of a multi-disciplinary team. Agreeing how we ‘providers’ do this together across the city is a specific aim of both the LES and DES.

This is a virtual session scheduled for Thursday 26th May 2022, 1-2pm and is open to all staff working in primary care who have an interest in MDT development. There is no need to book. Please add the meeting details to your own calendar and contact [email protected] for the meeting invite.
