Confed Connects – Frailty

Leeds GP Confederation is launching new, informal, drop-in sessions called Confed Connects. Open to anyone working in any role in general practice, these sessions will cover a range of topics and will support you with latest guidance, clarify implications of local decision making for practices and PCNs, and step through practical implementation. There will be chance to ask questions and share your views on what you hear.

Our first Confed Connects session will focus on frailty and is scheduled for 21st April 2022, 1pm – 2pm. Dr Chris Mills is hosting and will outline his work with health and care system partners as Chair of the Leeds Frailty Board, operational updates relevant to general practice, and key upcoming decisions.   The session will combine Confed Connects with the frailty learning lunch session mentioned at the members meeting so will be joined with CCG colleagues to discuss the next phase of the enhanced frailty scheme and how this links to the frailty board.

There’s no need to book and you can join the session. Please contact [email protected] for the meeting link.

We look forward to connecting with you more regularly, sharing what’s going on, why it matters to practices and PCNs, and hearing from you.
