We will be holding the next quarterly Primary Care: Care Home Scheme & Enhanced Frailty Peer Support Group on Wednesday 20th September, 12:30 – 14:00.

The September agenda will include :-

  • Polypharmacy: Frail and older people – Heather Smith, Consultant Pharmacist: Older People
  • Quality Improvement: Falls in Care Homes – Dr Ellen Oakhill, GP
  • Update ON the Falls Pathway Refresh work – Jo Brayshaw – Falls ACP/Pathway Lead
