We will be holding the next quarterly Primary Care: Care Home Scheme & Enhanced Frailty & Peer Support Group on Wednesday 22nd March, 12:30 – 14:00
This is a session aimed at those with an interest in frailty and enhanced health and care homes, and those delivering frailty and enhanced health in care homes services
The March agenda will include :-
- Enhanced Frailty Scheme – Promis/PC3 update
- I-Stumble care home pilot and training for care homes
- Improving systems between the LYPFT Care Home Team & General Practice
- Anticipatory Care Update
