Introducing Active Recovery

Active Recovery aims to create a health and social care short term community rehabilitation and reablement service for Leeds.
This new programme involves combining the resources of Leeds City Council (LCC) SkILS Reablement Service and Leeds Community Healthcare (LCH) Neighbourhood Teams to create a multi-disciplinary service delivery model.
We want to work with partners and hear your ideas on how we can best deliver our services to meet the needs of the people we support and our workforce, building on what works well already.
