We will be holding the next quarterly Primary Care: Care Home Scheme & Enhanced Frailty & Peer Support Group on Thursday 15^th September, 12:30 – 14:00.

The September agenda will include the following :-

Meds Optimisation and SALT colleagues will be joining us to talk about dysphagia and support to people with swallowing difficulties.

Sean Ninan, Community Geriatrician will update us the development of a pilot training package for frailty

An update on phase 2 of the Enhanced Frailty Scheme and sharing experience of working in a Frailty team
