Leeds Heart Failure Education and Pathways. 29th June 13:00-16:00

We would like to invite you, or any interested parties to an online event to launch the new Leeds Heart Failure Pathways and present the development of the Leeds Integrated HF service. This is an event presented by Dr Kate Gatenby and Dr Alex Simms, clinicians from Leeds Teaching Hospitals, supported by Clinical Pharmacists from the Beeston and Middleton PCNs, Heart Failure nurse specialists from Leeds Community Healthcare, representation from Leeds CCG and supported by Leeds GP confederation.

The event is for anyone working in healthcare in Leeds (and beyond!) who would like to understand how we might manage patients with heart failure in the future. We are hoping the event will be attended by clinical pharmacists, practice nurses, GPs, ACPs, clinical leads of PCNs or managers who might wish to understand why this is important and how we can achieve it.

The session, to be delivered via an online platform is scheduled for the afternoon on the 29th of June.

The session is designed to run through the recently launched new Heart Failure Pathways, and to present the results of a pilot project to educated clinical pharmacists in the community to optimise the medical therapy for patients with heart failure. It will include a live MDT between consultant cardiologists, heart failure nursing specialists and Primary Care pharmacists/practice based pharmacists and is intended to show how this support might be delivered on a city wide footprint.

We hope that it will provide some insight into how the new CCSP template and heart failure pathways can be used to complete HF chronic disease reviews in primary care and demonstrate the level of support which secondary care hope to be able to provide in the long term to help better manage patients with heart failure across Leeds.
