GP Extended Access Service

APMS contract commissioned by NHS Leeds Clinical Commissioning Group (CCG)

Our GP extended access service is currently provided from 12 hubs across the city. The service operates Monday to Friday 6.30am – 8.00 pm and Saturday and Sunday mornings. We are contracted to provide the service seven days per week, 365 days per year.

Between 1 April 2018 and 31 March 2019, patients across Leeds benefitted from an 103,599 appointments being made. 82,106 patients were seen in the service (87.6% take up) and the average missed appointment (DNA) rate for the year was 8%.

Appointments are available with a range of primary care professionals including GPs, nurses, health care assistants, clinical pharmacists, physiotherapists, and phlebotomists.

Day to day management of the citywide service is provided by the Confederation operational management team.

NHS Health Checks

Ccommissioned by Leeds City Council

This is a three year contract with the option to extend for a further two years that started on 1 April 2019. The head contract is held by Leeds GP Confederation with the Council. We have subcontracted delivery of NHS Health Checks to all practices in Leeds.

NHS Health Checks remain a ‘universal offer’ for anyone aged 40 to 74, who are eligible, and have not had an NHS Health Check in the last five years. However, as part of the key performance indicators, the council has asked that people who are economically disadvantaged (often referred to as deprived), smokers or those that are obese [BMI > 30] are targeted as these are the populations most likely to benefit. The total eligible population across the city is 229,273 with 20% expected to be invited each year.

The citywide target for 2019/2020 is to undertake NHS Health Checks on 51% of the target population for the year which equates to 23,386 completed NHS Health Checks. Of these at least 50% should be undertaken on patients who are deprived, smokers or obese i.e. 11,693. To help practices to increase uptake, dedicated NHS Health Check appointments with HCAs will be rolled out in the extended access hubs.
